The pilot is the fruit of more than four years of planning and collaboration among health care providers in the Adirondack North Country. It developed from the pressing need to increase support for primary care, and to address a growing and potentially critical shortage of primary care physicians.

The Adirondack region has seen a steady and growing decline in the availability of doctors in recent years and has the highest patient-to-primary-care-doctor ratio in New York State.
