The pilot is organized as three geographic “pods,” the Northern Adirondack (Plattsburgh and surrounding communities), Tri-Lakes (Saranac Lake and surrounding communities), and Lake George (Lake George and surrounding communities).
A governance committee of participating payers and providers, chaired by an official of the New York State Department of Health, has the following responsibilities:
- Monitoring the progress of practices and making initial and continuing participation decisions when necessary
- Making decisions regarding new applications for participation from providers/practices
- Resolving attribution disputes (if necessary)
- Enforcing terms and conditions
- Assessing overall pilot performance
- Clinical/quality performance
- Financial performance
- Developing process and procedures for data access
- Ensuring regulatory compliance
- Managing grants/funds
- Meeting logistics
- Communications/education
- Internal (participants)
- External (press, interested parties, employer groups, etc.)
- Compensation and participation requirements (terms and conditions)
- Oversee any changes or revisions needed
- Pay for performance
- Acting as liaison with other governance entities or projects including (but not limited to) Rural Health Networks and HEAL 10
- Creating and defining subcommittee activities
